Forms & Policies


Listed below is a brief list of commonly used forms at Cal Poly including where to obtain them, as well as general instructions for the student to initiate the process. Keep in mind that, depending on the situation, forms may take up to the length of one quarter to be processed. To check on the status of the form, contact Orfalea Student Services at 805-756-2601 or cobadvis@calpoly.edu.

These processes are specific to the Orfalea College of Business. If a course is not offered within the Orfalea College of Business, the corresponding form should be submitted to whichever department the course is being taken within.

Description: Allows a student to declare or change their concentration within the Orfalea College of Business.

Declaring (or changing to) Accounting: Obtain form at Orfalea Student Services (03-100) or download it as a PDF. You must meet with either Doug Cerf (dcerf@calpoly.edu) or James Kaney (jkaney@calpoly.edu) to obtain a signature. Then, submit form to Orfalea Student Services.

Declaring (or changing to) any other concentration: Complete an online form.

Time-frame: These forms typically take 4-6 weeks to process before showing up in your Cal Poly portal.

High-volume dates: Ongoing, influx in submissions in Spring Quarter

Description: Allows a student to move to an earlier or later graduation date after already having been assigned an expected graduation date (shown in Poly Profile).

Obtain at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222) or download it as a PDF.

If moving to an earlier graduation date: Submit form to Orfalea Student Services (no meeting required).

If moving to a later graduation date: Meet with an Academic Advisor at Orfalea Student Services.

High-volume dates: Ongoing, or generally around registration time (week 6, 7, 8, 9)

Description: Allows a student to withdraw from a course after the add/drop deadline.

Obtain form at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222)

Next steps: Obtain a signature and comments from the Professor of the course, then meet with an Academic Advisor at Orfalea Student Services.

Requirements: There must be a serious and compelling reason for withdrawing from the course that occurred after the add/drop deadline of the quarter. View the complete requirements from the Office of the Registrar on their Withdrawal Procedures page.

Deadline: End of week 7 of the quarter (Friday, 5pm)

Description: Allows a student to withdraw from all of their courses for a term within the first seven weeks of the quarter.

Obtain at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222)

Next steps: Meet with an Academic Advisor at Orfalea Student Services.

Requirements: There must be a serious and compelling reason for withdrawing from the term that occurred after the add/drop deadline of the quarter. View the complete requirements from the Office of the Registrar on their Withdrawal Procedures page.

Deadline: End of week 7 of the quarter (Friday, 5pm)

Description: Allows a student to register for more than 22 units in a single quarter.

Obtain at: The Office of the Registrar (01-222) or download it as a PDF.

Next steps: Get form signed by an Academic Advisor at Orfalea Student Services, then submit form to the Office of the Registrar no later than 5pm on the 8th day of instruction.

High-volume dates: Generally around registration time (week 6, 7, 8, 9)

Description: Allows a student to retroactively withdraw from a previous quarter for extenuating circumstances out of the student’s control.

Obtain at: the Office of the Registrar (01-222)

Next steps: Meet with an Academic Advisor at Orfalea Student Services.

Requirements: Must include a student statement and supporting documentation of the extenuating circumstance. View the complete requirements from the Office of the Registrar on their Withdrawal Procedures page.

Deadline: End of term one academic year after initial quarter was graded

Description: Allows a student to not be enrolled in courses at Cal Poly for 3+ consecutive quarters (not including summer) due to reason that contributes to the student’s educational objective.

Obtain at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222) or download it as a PDF.

Next steps: Meet with an Academic Advisor at Orfalea Student Services to get a signature.

High-volume dates: Ongoing, typically not time sensitive

Description: Allows a student to not be enrolled in courses at Cal Poly for 3+ consecutive quarters (not including summer) due to a military necessity. This leave may be extended for up to 5 years.

Obtain at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222) or download it as a PDF.

Next steps: Submit the form to Orfalea Student Services (no meeting is necessary). A copy of military orders is required.

High-volume dates: Ongoing, typically not time sensitive

Each of the 7 minors within the Orfalea College of Business have different requirements for declaring. Additionally, visit the Cal Poly Minors page for a comprehensive list of all minors offered at Cal Poly.

Description: Allows a student to request a deviation from the published curriculum for a Major/Support course within the Orfalea College of Business.

Obtain at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222).

Note: It is recommended to discuss the likelihood of the substitution being approved with the chair of the corresponding department.

Time-frame: These forms typically take 6-8 weeks to process before showing up in your Cal Poly portal. It is best to submit this form before enrolling in a course that you are hoping to substitute.

High-volume dates: Ongoing, typically not time sensitive

Description: Allows a student to withdraw from a course after the end of the 7th week of the quarter.

Obtain form at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222)

Next steps: Obtain a signature and comments from the Professor of the course, then meet with an Academic Advisor at Orfalea Student Services.

Requirements: There must be an emergency reason for withdrawing from the course that occurred after the end of the 7th week of instruction. View the complete requirements from the Office of the Registrar on their Withdrawal Procedures page.

Deadline: End of week 10 of the quarter (Friday, 5pm)

Description: Allows a student to withdraw from all of their courses for a term after the end of the 7th week of the quarter, until the last date of the quarter.

Obtain at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222)

Next steps: Meet with an Academic Advisor at Orfalea Student Services.

Requirements: There must be an emergency reason for withdrawing from the term that occurred after the end of the 7th week of the quarter. View the complete requirements from the Office of the Registrar on their Withdrawal Procedures page.

Deadline: End of week 10 of the quarter (Friday, 5pm)

Description: Allows a student to request to move to a different catalog.

Obtain at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222) or download it as a PDF.

Next steps: Submit form to the Office of the Registrar.

Requirements: Note that you may only change to a newer catalog, not an older one. For example, if you came to Cal Poly on the 15-17 catalog, you may switch to the 17-19 catalog, but NOT to the 13-15 catalog.

High-volume dates: Ongoing, typically not time sensitive

Description: Allows a student to participate in a commencement ceremony that is different than the one that they are scheduled to participate in (shown in Poly Profile).

Obtain at: the Cal Poly Commencement Website

High-volume dates: Leading up to commencement (weeks 6, 7, 8 and 9 of Spring and Fall quarter)

Description: Allows a student to enroll in a course after the add/drop deadline.

Obtain at: The Office of the Registrar (01-222)

Next steps: Obtain a signature from the Professor offering the course, then submit the form to Orfalea Student Services (03-100). Keep in mind, you may be subject to a $25 late registration fee.

High-volume dates: Generally around start of quarters (week 1, 2)

Description: Allows a student to not be enrolled in courses at Cal Poly for 3+ consecutive quarters (not including summer) due to a medical necessity. This form may be submitted retroactively.

Obtain at: Orfalea Student Services (03-100) or the Office of the Registrar (01-222) or download it as a PDF.

Next steps: Submit the form to Orfalea Student Services (no meeting is necessary). A written letter together with medical documentation is required.
