As a student at Cal Poly under the Orfalea College of Business, you need to abide by certain policies in order to graduate. These guidelines are here to help motivate you, and to help keep your path in the right direction through your college career.
We have listed the most commonly questioned policies here for your convinience, but be sure you come into the Advising Center if you have any further questions.

Request for Graduation Evaluation
Use this form to request your graduation evaluation during the fourth quarter prior to your expected degree completion term. This form must be filled out and signed by student.
Student Scheduling Worksheet
Use this worksheet to plan out your courses over multiple courses. This is the same worksheet that a Peer Advisor would use when helping you plan out a schedule.

Change of Degree Completion Date
If degree will not be completed by completion date stated on Graduation Evaluation, then this form should be filled and signed by student.
Choice of Catalog (sample)
Cal Poly issues a new catalog every one or two years, and the requirements for degree programs may change from one catalog to the next. Students have rights to the catalog year upon entry into college or any year thereafter. This form needs to be completed if a student wishes to choose a catalog other than the one they entered Cal Poly under. You can receive this special form from the Advising Center.
Course Withdrawal Petition
Student may withdraw from a class only if they have a serious or compelling reason and appropriate documentation. The student, instructor, Academic Advisor, and Area Chair offering this course must sign the form in that order. Please read the OCOB Withdrawal Policy before picking up a withdrawal form in the OCOB Advising Center.
Excess Unit Request
Undergraduates requesting more than 20 units or Graduates requesting more than 16 units for a quarter needs to fill in this form.
Grade Change
This form is used to change a grade. Please know that you must have an instructor's signature in order to change a grade. You must initiate this with the instructor.
Major Change/Drop Request
This form is used to drop your previous major and then add your new major once accepted. Please pick this form up at the Office of Academic Records. Be sure to bring your acceptance letter with you if adding a major.
Permit Request
Students can ADD a closed class by showing up on the first day and attempting to crash the course. Instructors may have different policies for adding a course. The student must fill out this form, sign it, and get instructor's signature to save a seat in the class. However, they still need to enroll on POWER and CAPTURE to be officially enrolled in the class.
Substitution Forms
- General Education & Breadth Substitution
- Use this form to petition to have a different class count as a general education requirement. Please pick this form up from the Advising Center.
- Major or Support Substitution
- Use this form to petition to have a different class count as a major or support course requirement. Please pick this form up from the Advising Center.
- Concentration Substitution
- Use this form to petition to have a different class count as a concentration course requirement. Please pick this form up from the Advising Center.
Time Conflict Request
This form is used when you need to take classes whose times conflict. This should be used as a last resort. This form can be picked up at the Office of Academic Records.
Transcript Request
Student can request transcripts by filling out this form and paying the fees.
