As a student at Cal Poly in the Orfalea College of Business, you need to abide by certain policies in order to graduate. These guidelines are here to help motivate you, and to help keep your path in the right direction through your college career. We have listed links to the most commonly questioned policies here for your convenience, but please see the Catalog for University Policies and the Office of the Registrar website for a complete list of policies and forms.
Listed are some commonly requested forms that can be found in our office. If not listed here, you will need to visit the Office of the Registrar (Bldg. 01, 2nd Floor) to obtain the necessary form.
Request for Graduate Evaluation
Use this form to request your graduation evaluation three to four quarters prior to your expected graduation date.
Commencement Petition Request
If you would like to participate in a commencement ceremony that does not correspond with your date of graduation, you will need to fill out this form.
Change of Degree Completion Date
If you will not be able to complete your degree by the completion date stated on your Graduation Evaluation, then you will need to fill out this form.
Change of Catalog Request
This form is for students who want to change their catalog year to a catalog that they have rights to. For instance, a student on the 07-09 catalog could change to the 09-11 catalog if they wished. We recommend that you come in and talk to the Advising Center if you are thinking about changing your catalog.
Choice of Catalog
Students may request to complete their degrees on a catalog older than 10 years if the only remaining degree requirements at the time they left Cal Poly do not exceed 16 units. These remaining degree requirements may include senior project, GWR, and/or USCP.
The decision to approve or deny a student’s request is based on both:
- her/his willingness to commit to completing outstanding degree requirements within a specified time frame and
- her/his ability to demonstrate, with written documentation, reasonable currency of knowledge and skills in her/his degree field to the satisfaction of the faculty in the applicable major, as certified by the department chair. Both the college dean and the Vice Provost for Academic Programs & Undergraduate Education must give their approval.
Course Withdrawal Petition
Student may withdraw from a class after the add/drop date only if they have a serious or compelling reason and appropriate third-party documentation. The student, instructor, Academic Advisor, and Area Chair offering this course must sign the form in that order. Please read the Orfalea College of Business Withdrawal Policy before picking up a withdrawal form from the Advising Center.
Excess Unit Request
Undergraduates requesting more than 20 units or Graduates requesting more than 16 units for a quarter need to fill out this form.
Use this form to petition to have a different class count as a major/support/concentration course requirement. You will need to fill out this substitution form in the Advising Center.
